Chelsea and Westminster Flagging System and Hospital Passport:
The flagging system at Chelsea and Westminster hospital was introduced to make it easier for children and adults with Learning Disabilities and their families to access the Healthcare Provision at the Hospital. Developed in partnership with Full of Life, the system highlights to Healthcare Staff that the person has a Learning Disability and allows parents / carers or the young person themselves to provide more specific information about their individual needs. This could include their Communication and Sensory Needs or whether they need a specific appointment. By signing up to this system the person’s medical file will be flagged so that anytime it is accessed this information will be made available to the Healthcare Staff, providing them with information which should help provide a more positive experience for both you and your child.
The Hospital Passport is a portable document which can be carried with your child or adult to Healthcare Appointments and given to practitioners before appointments to get a more in depth view of the person and their needs. To download a copy of the passport click here
