NHS Continuing Care is support provided for Children and Young People under 18 who require a specific package of care due to Complex Health Needs. Professionals working with a Child or Young Person may offer them an assessment if they believe that they could have Continuing Care needs.
They can arrange a Holistic Assessment, which may include other services such as Education and Social Services. The assessment process is carried out by a Children and Young People’s Health Assessor. Reports may be from services already involved with your child. The Assessment considers what daily support a child might need with:
- Mobility
- Nutrition, food and drink
- Continence and elimination
- Behaviour that Challenges
- Communication
- Skin and tissue viability
- Breathing
- Drug therapies and medicines
- Psychological and emotional issues
- Seizures
A decision on whether your child is eligible should be provided within 28 days of the referral being received. If it’s decided that the Child or Young Person is eligible for ‘Continuing Care’, then provision can be made by Health, Education and Social Services.
