Once you have contacted Social Services, they will refer your young adult to an appropriate team for a needs assessment, which looks at their care and support needs. There are three main teams which support adults with additional needs in RBKC, with each team specialising in a different set of support needs. They can offer assessments and packages of care and support in order to help adults live as independently as possible.
Learning Disability Team
The Learning Disability team supports adults with Learning Disabilities. The team provides a multi-disciplinary service made up of both health and social care workers. This includes social workers, care managers, community nurses, occupational therapists, physiotherapists, psychologists, speech and language therapists and psychiatrists.

Adult Social Care Team
